AMGA Value Care Network

Achieving better patient outcomes

AMGA Value Care Network has entered into a collaborative agreement with CMS to improve the quality of care for our patients while working to reduce healthcare expenditures. Our providers work together in a coordinated manner, focusing on preventative care, care coordination, and population health management to transform the healthcare delivery system. Through our participation in the Medicare Shared Savings Program with the Centers for Medicare and Medicaid, we aim to achieve better patient outcomes, provide more efficient care, and meet our cost-saving targets.

About Us

The Department of Family and Preventive Medicine within University of Utah Health is dedicated to advancing health and well-being across the lifespan through comprehensive patient-centered care, education, and research. Our multidisciplinary team of physicians, advanced practice clinicians, educators, and public health professionals works collaboratively to deliver high-quality primary care, promote preventive health strategies, and improve population health outcomes.

As part of our commitment to value-based care, we are proud to participate in the Medicare Shared Savings Program through our partnership with the AMGA Value Care Network. This collaboration supports our mission to enhance care coordination, reduce healthcare costs, and ensure that Medicare beneficiaries receive the right care at the right time—without unnecessary duplication of services or medical errors.

Together, we are building a healthier future for our communities by integrating evidence-based practices, leveraging data-driven insights, and fostering strong relationships between patients and care teams.
